Passport Application

Passport Application forms are Free to download and applicants are required to fill in all the mandatory fields to be able to download the form. An application will be rejected if details entered online differ from the documents submitted with applications.

When filling your application, please take time to read and understand the attached instructions before submitting application to avoid delays in processing. Note that all applications must be signed and stamped by a qualified witness. Submit your completed application form with the relevant documentations by hand or by post with the relevant application fees. All downloaded forms not submitted within 60 days will no longer be valid.
